Kindergarten Readiness - ASQ Surveys
Our goal is to ensure that every student is ready to enter Kindergarten. With that goal in mind, we are asking parents to provided information about their child as they enter Kindergarten this year. There are two questionnaires you are asked to complete. They are described below and are included with this letter.
The data from these questionnaires will help us in a few ways. First it will help our teachers understand your student and how he or she can learn best. Second, it helps us understand how we can improve our early education programs. Lastly, it helps the Kansas Department of Education understand how communities are doing with early education and Kindergarten across the state.
We ask that these two questionnaires be completed by September 20th. They can be done online (links shown below). The surveys can also be completed on paper and returned to your child’s teacher. You can download the paper copies below.

Questionnaire #1: ASQ-3
The Ages & Stages Questionnaire, Third Edition (ASQ-3), is a screening tool that will provide a quick check of your child’s development. The information you provide will be helpful in determining if there are community resources or services that may be useful for your child or your family.
Section 1. The first section of the ASQ-3 looks at five developmental areas. Each area has six questions that go from easier to more difficult. Your child may be able to do some, but not all of the items. Read each question and mark:
- Yes your child is performing the skill
- Sometimes your child is performing the skill, or
- Not yet, which tells us your child is not yet performing the skill.
Following is a brief description of the five developmental areas:
- Communication: Looks at language skills, both what he or she understands and what he or she can say
- Gross motor: Looks at large muscles, how your child uses his or her arms and legs, and movements and coordination
- Fine motor: Looks at your child’s hand and finger movement and coordination
- Problem solving: Looks at how your child plays with toys and solves problems
- Personal-social: Looks at your child’s self-help skills and interactions with others
Section 2. The Overall section asks important questions about your child’s development and any concerns you may have about your child’s development. Answer questions yes or no, and if indicated, please explain your response.
Questionnaire #2: ASQ-SE-2
The Ages & Stages Questionnaires: Social-Emotional, Second Edition (ASQ:SE-2) is a screening tool that will provide a quick check of your child’s social-emotional development. This questionnaire asks questions about your child’s behaviors and social-emotional growth. The information you supply will help reveal your child’s strengths, uncover any areas of concern, and determine if there are community resources or services that may be useful for your child or your family. If completing this questionnaire brings up any questions or concerns, we can talk about information and resources that can help.
Please keep these tips and directions in mind while completing ASQ:SE-2.
Tips for completing ASQ:SE-2:
- Caregivers who know the child well and spend more than 15–20 hours per week with the child should complete ASQ:SE-2.
- Answer based on what you know about your child’s behavior.
- Answer questions based on your child’s usual behavior, not behavior when your child is sick, very tired, or hungry.
Directions for completing ASQ:SE-2:
- Please read each question carefully and check the response that best describes your child’s behavior:
- Often or always: My child performs this behavior often or always.
- Sometimes: My child sometimes performs this behavior (not consistently).
- Rarely or never: My child rarely performs this behavior or has never performed the behavior.
- Concerns: Check the circle to the right of a question if the behavior is a concern.
- Some questions have blank spaces for you to provide examples of your child’s behavior. Please be sure to explain your response(s).
- The Overall section at the end of the questionnaire asks open-ended questions about your child’s behaviors. Answer questions by marking yes or no and provide an explanation for your responses.
