Below you may print required forms for students to have medication at school. Print and complete the forms ane return to the school office or the nurse's office.

Please bring orders and medication is original bottle to school.
If you would like your child to be allowed to have an over the counter medication, forms, PLEASE DOWNLOAD and send medication and the form to school.
IMMUNIZATIONS:
Kansas Statute 72-5209, states all children must be appropriately vaccinated. Please make sure your child's vaccinations are compliant with KS law.
